I was having the same problem and I knew I had solved it before but I could not remember how. THEN I REMEMBERED! I was using Microsoft Explorer and the "File" link in the "Manage Your Directory Tree" would never go active. I switched to Google Chrome and it worked instantly. I have not tried Safari, Opera or Firefox.
I had scanned and uploaded a book but I had forgotten to rotate the pages before the upload. I did that and uploaded a new version with a different name, then I deleted the old version, then finally I renamed the new version. Once that was done I went back and RE-DERIVED the file .... which is in process now. I hope it all works out!
